Patient Financial Responsibility
You, the patient, are ultimately responsible for all charges associated with your care. This includes, but is not limited to:
- Any outstanding balances not covered by insurance
- Co-pays, deductibles, and co-insurance amounts at the time of service
- Services deemed not covered or denied by your insurance company

Non-Participating Insurance Plans
If we do not participate with your insurance provider, you will be required to pay in full at the time of service. We will provide you with an itemized bill that you can submit to your insurance company for potential reimbursement.
Payment Methods
We accept cash, cards and checks. Payment is expected at the time of service unless prior arrangements have been made. For any balances due after insurance processing, we will send you a bill with the remaining amount, which must be paid prior to scheduled appointment.
